Being a Patient-Centered Medical Home (PCMH) means:
Care is Coordinated and/or Integrated
Care is facilitated by disease registries, information technology, health information exchange and other means to get care when and where it is needed and in a culturally and linguistically appropriate manner.
Whole Person Orientation
The primary care physician is responsible for providing all the patient's health care needs or arranging care with other qualified professionals. This includes all stages of life; acute care; chronic care; preventive services; and end of life.
A Primary Care Physician
Each patient has an ongoing relationship with a personal physician trained to provide continuous and comprehensive care.
Quality and Safety are Hallmarks of the Medical Home
Practices advocate for their patients to support the attainment of optimal, patient-centered outcomes that are driven by a compassionate, robust partnership between physicians, patients, and the patient's family or caregivers. Evidence- based medicine and clinical decision-support tools guide decision making; continuous quality improvement activities are enhanced through voluntary engagement in performance measurement; patients participate in decision-making and feedback is sought.
Patient Services Include:
Our office has a laboratory, digital x-ray, ultrasound Doppler and EKG. We are able to measure bone density and run pulmonary function tests. A private refill line is available and prescriptions are electronically sent to your pharmacy.
Physician Directed Medical Practice
The primary care physician leads a team of individuals at the practice level who collectively take responsibility for the ongoing care of patients.
